I want this to look like a professional certificate. This needs to be chalk full of information, but it needs to look like it has been issued by an authorized agency. Attached is a file that one of our competitor's sends to their customers. It is a half page, and I think it's really a simple, handsome little certificate that hits home exactly what we want to accomplish. If we can have something that looks similar, is a half sheet, has the same information, then I think we'll be happy. At the top of the information section, I'd like to have the words, "This is to certify that Alarm Grid is monitoring the security system at:" The fields I want are the following: * Customer Name * Street Address Where System is Installed *City / State / Zip Code (Same as the red certificate that is included) Remove the System Installation Date and instead put in, "Signals Last Tested." Underneath the customer's information include the following with checkmarks next to it. "The Following Services Are Being Provided:" * Intrusion * Smoke or Fire * Carbon Monoxide * Water * Temperature Right below that, add a section and add checkmarks next to each option: "This System Uses the Following Communicators * Phone Service * IP or WIFI * Cellular To the right, add the XFinity's little section. Make the heading "The system * Is * Is Not currently monitored by a UL-listed central alarm monitoring station." On the next line in small text: "Alarm Grid does not contact authorities for systems that are NOT connected to a UL-listed central station. Alarm systems are not a substitute for insurance. Warning devices and monitoring do not guarantee the prevention of " Below that, put the insurer's information like is in the .doc. "Company" Addresss Agent Name Agent Phone Number Agent Fax Number. Right below that, add in small text "This certificate is good for 1 year beyond the date indicated as "signals last tested." Put a small line for signature. Under the line mark it "Technician" On the far right, bottom corner, put down our name, address, and phone number.

What to avoid
I would like our newer logo without the grid background to be used. Also, no "DIY Security Solutions." The bottom of the document should not have "President" either. It should say, "Technician." These fields should not be filled in. This is going to be a pdf and will be filled in on behalf of customers.
